Arun Gulani, MD, is a board-certified ophthalmologist who has been treating Jacksonville LASIK and cataract surgery patients for more than a decade. The Goal of Refractive Surgery, Including LASIK

Dr. Gulani's has extensive expertise in a vast
spectrum of vision correction procedures.

Just like a camera, the eye has a view finder: the cornea. The shape of the cornea determines vision. While normal corneas are shaped like a basketball (spherical), nearsighted corneas are steep or more curved; farsighted corneas are flatter; and astigmatic corneas are shaped like a football (oval). Thus, nearly all cornea-based refractive surgeries (including LASIK) focus on re-shaping a patient's cornea.

Laser Vision Correction at Gulani Vision Institute

Laser vision surgery in every form — i.e., LASIK, wavefront LASIK, Custom LASIK, Epi-LASIK, LASEK, PRK, advanced surface ablation (ASA) — can be modified to achieve the desired visual outcome. As noted, Dr. Gulani offers a range of laser vision correction treatment options, including wavefront-guided all-laser LASIK surgery. This bladeless LASIK approach involves the use of a femtosecond laser in lieu of a mechanical blade to create a flap in the surface of the cornea. During the procedure Dr. Gulani gently lifts back the flap to access the underlying corneal tissue and uses an excimer laser to re-shape the curvature of the cornea, thereby correcting the refractive error causing nearsightedness, farsightedness and / or astigmatism. This often eliminates the patient's daily dependence on prescription eyewear. Patients who are not suitable candidates for LASIK may instead benefit from other vision correction treatment options, including PRK or implantable contact lens (ICL) surgery.

Cataract Removal at Gulani Vision Institute

The Jacksonville LASIK surgeon is also a talented cataract surgeon performing both traditional and laser cataract surgery. During laser cataract surgery, Dr. Gulani employs a laser to create a miniscule incision in the lens-encapsulating section of the eye. He uses a technique called phacoemulsification to break up the cataract-diseased eye lens into tiny fragments, which he then gently suctions from the eye. Next, Dr. Gulani replaces the removed lens with an intraocular lens implant (IOL) to restore clear vision. Gulani Vision Institute offers both conventional IOL options as well as premium IOLs. These include the Crystalens accommodating IOL as well as ReZoom and ReSTOR multifocal IOLs, which allow patients to see clearly at multiple distances. He also offers the AcrySof toric IOL to correct astigmatism.

Given the wide range of surgical approaches and lens technologies that are available, Dr. Gulani has access to a nearly infinite number of treatment combinations, allowing him to individualize vision correction treatment for each patient and help them achieve their unique visual goals. In this way, vision correction surgery might be considered a form of art.

Aside from offering patients in Jacksonville cataract surgery and LASIK surgery, Dr. Gulani provides treatment for presbyopia, keratoconus, pterygium, dry eye and more. In addition to performing corneal transplantation, he is also a widely renowned provider of ICL surgery. Again, thanks to Dr. Gulani's extensive expertise in a vast spectrum of vision correction procedures, he is able to choose the procedure — or combination of procedures — that is best for each specific patient.

For example, a patient with glasses and contact lenses may have a myriad of refractive problems, such as nearsightedness (myopia), farsightedness (hyperopia) or astigmatism. The patient might also have associated presbyopia (the need for reading glasses after the age of 40).

Additionally, an individual may have cataracts, and instead of LASIK, he or she may need cataract surgery with the use of advanced multifocal lens implants, such as ReStor, Crystalens, Tecnis or ReZoom. As mentioned above, femtosecond-laser-assisted cataract surgery is also available to those patients deemed to be appropriate candidates.

In some cases, LASIK may not be warranted. For instance, if the patient suffers from keratoconus, INTACS may be a better way to go, and can be combined with collagen cross-linking (CXL/C3R). Similarly, in cases of extreme nearsightedness, patients might require placement of the Visian ICL instead of LASIK surgery.

LASIK itself can be further customized based on the different technology options; variations of traditional LASIK include wavefront-guided LASIK, custom LASIK, bladeless LASIK, iLASIK, etc.

The vision of patients who have previously had RK (radial keratotomy) surgery can also be corrected. In addition, Dr. Gulani offers appropriate treatment options for patients with corneal scars, red eyes and dry eyes, which are often symptoms of disorders characterized by non-cancerous growths on the white part of the eye (sclera), such as pinguecula and pterygium.

About Arun Gulani, MD

Arun Gulani, MD, obtained his Doctor of Medicine degree at Seth G.S. Medical College / King Edward Memorial Hospital. He completed his ophthalmology residency at Interfaith Medical Center in Brooklyn, N.Y., before fulfilling an advanced training fellowship at Neumann Eye Institute in Florida. Dr. Gulani is a former chief of the Cornea, External Disease and Laser-Refractive Surgery department and assistant professor at the University of Florida School of Medicine. He has earned hospital privileges at a number of distinguished medical institutions in Jacksonville, Fla., including St. Luke's Hospital, Baptist Medical Center, St. Vincent's Medical Center and Memorial Hospital Jacksonville, among others.
